How to Get a Medical Cannabis Card in Vermont

Speak with a Physician

If you live in Vermont and have at least one qualifying medical condition, you are eligible for a medical marijuana card (MMJ card). Conditions must be diagnosed by an approved physician during a Vermont MMJ card appointment.

Which Conditions Qualify for a Medical Cannabis Card? 

There are several different types of conditions that may qualify for a medical cannabis card in the Green Mountain State, including: 

  • Cancer

  • Crohn’s disease 

  • Glaucoma (only when experiencing severe, persistent, and intractable symptoms)

  • HIV/AIDS

  • Multiple sclerosis 

  • Parkinson’s disease 

  • PTSD (requires a Mental Health Care Provider Form) 

  • Any disease, medical condition, or its treatment that is chronic, debilitating, and results in one of the following intractable symptoms: 

    • Cachexia (wasting syndrome) 

    • Chronic pain 

    • Severe nausea 

    • Seizures

Fill Out a Verification of Qualifying Conditions Form

During your MMJ card appointment, your physician will complete a Qualifying Condition Verification Form certifying that you have at least one of the aforementioned qualifying conditions. Keep this document in a safe place as you will need it in order to register with the Vermont Cannabis Control Board.

Submit Your Paperwork to the Vermont Cannabis Control Board

After obtaining your completed Qualifying Condition Verification Form, you’ll need to register with the Vermont Cannabis Control Board and complete their application. Both of these tasks can be performed online.

What Information You’ll Need When Registering

Make sure you have the following items on-hand when registering online: 

  • Your completed Qualifying Condition Verification Form 

  • Evidence of residency

  • A digital photograph

  • Your application fee 

Submitting a driver’s license is not sufficient enough to qualify as your digital photograph. All pictures provided must reflect your current physical appearance and feature your full face-and-shoulder profile without sunglasses or hats. 

As of March 1, 2022, all caregiver applicants are also required to obtain a fingerprint-based criminal background check. Applicants may be temporarily registered by the Board if they are a spouse, domestic partner, child, legal guardian, parent, sibling, grandparent, or grandchild of the patient. 

If you are a caregiver applying for an MMJ card, you will need to make an appointment with one of the 15 identification centers in Vermont. Prior to attending your appointment, the Medical Cannabis Program staff will provide an authorization form to be taken to the identification center. 

How Much Does a Vermont MMJ Card Application Cost? 

The initial application fee for a medical cannabis card in Vermont is $50. Card carriers will need to renew their application status once annually. It costs $50 to renew after 12 months and $50 per caregiver application. Along with your application fee, you can also expect to pay at least $200 for a consultation with an approved, licensed physician.
